			<content:encoded><![CDATA[<p>Medical tourism is a rapidly growing industry. Long NHS waiting lists and expensive private care have meant that 2.65 million Britons have recently been or are planning to go abroad for medical procedures.<br />
Gone are the days when going overseas for a medical procedure meant solely cut-price cosmetic surgery and a recovery in the sun. Now patients are opting to go abroad for cheaper deals on hip replacements, dentistry and infertility treatment.</p>
<p>Not that it is without controversy – a recent report in The Lancet warned that antibiotics&#8217; days were numbered because of global travel and medical tourism.<br />
According to currency broker Currencies.co.uk, one in 20 people have recently had a medical or dental procedure outside Britain – or are planning one. Sterling&#8217;s strength against eastern European currencies and NHS cutbacks have meant countries such as Bulgaria, Poland and the Czech Republic have seen increasing numbers of medical tourists.<br />
With spending cuts expected to hit the NHS, more people might be tempted to look overseas. So how do you ensure that you save without risking your health?<br />
A spokesman for the British Medical Association (BMA) said thorough research was essential before any patient considered going abroad for treatment.<br />
&#8220;It is extremely important that patients consult their GP for advice. Patients should also investigate all aspects of their proposed treatment. This must include the health and safety standards of the facilities and the potential impact of long-distance travel on their recovery from all forms of medication or surgery abroad,&#8221; he said.<br />
There are several ways to check the standard of care. Dr Edward Watson, a former accident and emergency doctor, said there was no global regulatory body for medical travel, but there is a universal body for accreditation – ISQua (the International Society for Quality in Health Care). Dr Watson set up Medtral New Zealand for Americans looking for more affordable treatment.<br />
&#8220;ISQua has members in 70 countries, including Canada, the UK, Ireland, New Zealand, Australia and South Africa, as well as Japan and Taiwan,&#8221; he said.<br />
&#8221;You should also check your surgeon&#8217;s training, patient testimonials and published &#8216;adverse event rates&#8217;. You have to be careful with these as they are not independently verified and some hospitals we find do lie. Also, some hospitals talk about their overall adverse event rate. If they do thousands of eye operations and endoscopies then their overall event rate is very low compared to a hospital that does a lot of complex surgery.&#8221;<br />
There are also international hospital accreditation programmes. The Joint Commission International (JCI) accredits hospitals, while QHA Trent, a British company, accredits and provides consultancy services for hospitals and clinics globally.<br />
Many hospitals in India and other parts of Asia will have doctors who trained in Britain or America. Not only are their qualifications easier to verify, but they are more likely to speak good English. Check the level of English spoken at the hospital or aftercare facility – communicating with nurses can be paramount.<br />
As well as cosmetic and elective surgery, it is increasingly common for patients to go abroad for addiction therapy, obesity treatment, fertility treatment and hair transplants.<br />
According to figures based on the Medical Tourism Survey on behalf of Treatment Abroad, 43pc of British patients travelled abroad for dental treatments, 29pc for cosmetic surgery and 28pc for other items such as orthopaedic surgery and infertility treatment.<br />
With dentistry, although you can save £170 having a crown in Spain (36pc cheaper than in Britain), it will cost you £33 more to have a dental X?ray, which you will need before and after treatment. This loss can be recouped when considering the cost of an oral examination and consultation – £45 in Britain but free in Spain.<br />
It is also vital that you ensure that any aftercare you require is included in the package, or that you have made arrangements for yourself. The BMA considers this to be as important as the treatment itself. &#8220;Patients should guarantee that their treatment includes clear provisions, agreed with their overseas clinicians, for follow-up care after any medical procedures,&#8221; it said.<br />
Booking through an agent or operator will mean that you can negotiate all-inclusive packages. Although more expensive, you are more likely to receive a higher quality of care before, during and after a procedure.<br />
TreatmentAbroad.com provides information for patients about clinics, hospitals and specialists worldwide that are attracting medical tourists. Keith Pollard, director of Treatment Abroad, said you should think about how much time you will need for rehabilitation and whether you wish to have a friend or partner support you.<br />
He advised patients to ask operators for a breakdown of costs and to provide them with as much detail as possible about your needs, including medical history, dietary requirements and allergies. Mr Pollard warned that many patients made the mistake of thinking their annual travel insurance policy would cover elective surgery abroad – just because it covers an accidental occurrence that leads to them requiring medical treatment.<br />
Jennifer Thomas, from Direct Line Insurance, added: &#8220;Our policies do not provide cover for medical costs if a patient has elected to go abroad for treatment. This includes any costs relating to complications arising from the elected surgery. The main purpose of the cover in our policies is for emergency medical situations arising from accidental occurrences.<br />
&#8220;However, should a customer choose to take a holiday for rest and recuperation after the elected surgery, and they have an unrelated accident or illness, cover is provided. Cover would also be provided in the event of accident and illness in the days leading up to the elected treatment but not while directly on the way to the hospital or clinic.&#8221;<br />
Patients should opt for a specialist &#8221;enhanced medical&#8221; insurance policy such as those provided by Seven Corners (www.seven corners.com), Health Traveller (www.health traveller.co.uk), P J Hayman (www.freespirittravelfor treatment.com) and Angelis (www.angelisgroup.com)<br />
Medical tourism will continue to be on people&#8217;s agenda in time of economic hardship and NHS cuts, but anyone considering treatment overseas needs to consider it very carefully: it is a step into the unknown.</p>

			<content:encoded><![CDATA[<p>According to reports voiced at a recent Tourism Authority of Thailand (TAT) seminar in Qatar Thailand currently holds a 38% stake in the world’s medical tourism market, the largest of any country in the globe.</p>
<p>Qatar, the 5th of October 2010: Dr. Surapong Ambhanwong (Phayathai Hospital Chief Medical &#038; International Business Officer) explained that the passage of international patients was currently producing an estimated 920,000 travellers to the Kingdom of Thailand annually, making it the number one destination for medical tourists.</p>
<p>Speaking at the inaugural Medical Health and Wellness Roadshow in Doha, Qatar, Dr. Ambhanwong explained that the scope of procedures being undertaken in Thailand was wide, covering areas such as plastic surgery, joint replacement, cancer treatment, open heart surgery and dental care.</p>
<p>“Many also visit Thailand to seek professional advice for a better quality of life, through Thailand’s many alternative medical centres and hospitals, to help control and reduce the threat of diabetes, cholesterol, obesity and blood pressure,” he said.</p>
<p>The roadshow, organised in conjunction with the TAT, was aimed at promoting Thailand’s expertise and specialisation in providing cutting-edge medical care throughout the world and in particular focusing on the Gulf States.</p>
<p>The initiative will see representatives from Thailand’s leading hospitals and wellness centers visit the Middle East to meet directly with industry professionals and medical travel consultants as well as regional government administrators.</p>
<p>Present at the roadshow, TAT director Pramoth Supyen revealed that Middle Eastern travel to Thailand was constantly on the rise, whether seeking medical care or leisure travel. He stated that the TAT was now focusing its aims on further promoting tourism in the region based on the success of initial cooperation with region governments.</p>
<p>“Thailand proves an attractive option as a medical destination due to the country’s well established industry to provide high quality, cost effective treatment of international standards,” he said “the medical industry has proven itself with the added advantage of providing far superior nursing care, as well as the unique Thai approach and attention to personal care”.</p>

<p><span id="more-538"></span>For others, it calls to mind hospital beds, surgeries and doctors.</p>
<p>Medical tourism — in which patients travel to other countries for medical procedures — has,  in some form, been around “for centuries,” said Jonathan Edelheit, CEO of the Medical Tourism Association, an organization that helps promote international medical travel.</p>
<p>But over the past five years, advances in technology and medicine combined with growing health care costs in the United States have caused interest in the practice to skyrocket, with no signs of slowing down.</p>
<p>“It will only become more common in the future,” Edelheit said.</p>
<p>Procedures performed in Israel cost, on average, 50 to 70 percent less than they do in the United States. Bypass surgery in Israel costs approximately $35,000 — in the United States, the same procedure runs about $120,000.</p>
<p>For the more than 15 million Americans who are unemployed and the 46 million without health insurance (as of 2008, according to the U.S. Census Bureau), those kinds of savings may be the only impetus necessary.</p>
<p>“Uninsured patients are looking for good and quality medical treatment all over the world, for a better price,” said Natalie Steiner, vice president of marketing for Global Health Israel, an organization that works to promote medical tourism in Israel.</p>
<p>Israeli doctors, hospitals and universities have also become highly respected both in the medical community and among patients, through the development of new technology and Israel’s state-of-the-art facilities.</p>
<p>“The reputation of the Israeli doctor is quite good all over the world,” said Eliezer Hod, marketing development director for the Israel Ministry of Tourism.</p>
<p>And as patients begin to experience success, word travels quickly through the community. “People often learn about medical tourism through word of mouth,” Hod said.</p>
<p>The vast increase in online access to information about medicine through the Internet has also played a big role in driving patients overseas in recent years.</p>
<p>Many people already rely on the Web for medical information when they can’t reach their provider, don’t have health insurance or are simply looking to become more informed. With Web sites making international doctors seem as close as your living room, trust builds and the idea of seeing a doctor in another country seems less questionable.</p>
<p>“The Internet is very strong in promoting medical tourism. I believe that this tool will become even more strong in coming years,” Steiner said.</p>
<p>To arrange for procedures, patients are connected directly with a specialist or hospital after contacting a tourism office or an organization such as the Medical Tourism Association. They will either be provided with a medical tourism facilitator, who acts as a middleman between patient and doctor, or have their contact information given directly to Israeli providers.</p>
<p>“We send the proposal to a few people in Israel who specialize in these operations, then they respond directly to the client,” Hod said.</p>
<p>For the most part, facilitators do not charge patients, but rather work off commissions from Israeli hospitals.</p>
<p>Both public and private hospitals in Israel accept international patients, with some, such as Hadassah Medical Center in Jerusalem and Assuta Medical Center in Tel Aviv, making a concerted effort to recruit patients from all over the globe.</p>
<p>Services available to foreign patients encompass nearly every specialization of medicine, ranging from cancer treatment and orthopedics to cosmetic surgery and weight loss procedures, like gastric bypass. Some of the most commonly sought procedures among Americans traveling to Israel are oncology and heart surgery.</p>
<p>But the most popular procedure, and the one that tourism officials and representatives hope to push to the United States market, is in vitro fertilization (IVF).</p>
<p>Unlike most countries, the Israeli government covers IVF procedures for two children for couples who are having difficulty conceiving. Their success rate is, by some measures, as much as 15 percent higher than other parts of the world. And the cost difference is staggering: an IVF cycle in Israel costs between $3,000 and $4,000, as compared to $16,000 to $20,000 in the United States.</p>
<p>“Because Israel provides IVF free to residents, they have much more expertise and better outcomes than other countries,” Edelheit said.</p>
<p>The full length of a patient’s stay in Israel varies depending on the procedure, but is generally about two weeks, Edelheit says. Some patients may opt to stay in a “recovery resort,” a place designed specifically to cater to those who are recently post-operative.</p>
<p>Through these extended stays, and the fact that most patients do not travel alone, the benefits of medical tourism extend beyond the patient.</p>
<p>“It has a positive effect on the local economy,” Edelheit said.</p>
<p>Adds Steiner, “The money that we get from medical tourists we can use to develop our medicine, to buy more medical equipment and to become more” technologically advanced.</p>
<p>Despite a thriving medical community and what looks to be a bright future for medical tourism, there are still reminders of the political tension in the area. A brochure released from Israel’s Office of Tourism in Los Angeles that focuses on pediatric care offers the following description of its services: “As the only purely rehabilitation hospital in Israel catering for children, Alyn specialized in rehabilitation<br />
techniques for conditions such as trauma and head injuries from terrorist attacks. … ”</p>
<p>Medical tourism is not currently covered by most insurance companies in the United States, but some are looking to incorporate the option into benefit packages.</p>
<p>“Companies that have a lot of Jewish [employees] or Israeli people have a reason to offer medical treatment in Israel,” Steiner said.</p>
<p>Technological advances will, she adds, shape the future of medical tourism, as doctors and patients become more comfortable communicating via the Internet and as health care becomes global.</p>
<p>“I believe that in few years you will find consultations via the Internet,” Steiner said.</p>
<p>For now, American patients will have to be content with capitalizing on the benefits of medical tourism – and maybe get a vacation out of it, too.</p>

			<content:encoded><![CDATA[<p><strong>As Congress wrangles to craft meaningful health care reform to deliver to President Obama by mid-summer, consensus has emerged on three fronts: reform is no longer an option, the economy is inextricably linked to reducing the cost of health care, and whatever form the overhaul takes will require years, perhaps decades, to implement and fund.</strong></p>
<p><span id="more-469"></span>Meanwhile, those who pay the bills—consumers and employers&#8211; are not waiting idly to see how legislators fix the world&#8217;s most expensive and least equitable health care system. They&#8217;re taking matters into their own hands, finding ways to reduce health care costs without sacrificing quality.</p>
<p>One of the solutions they&#8217;re turning to is medical tourism, which can be seen as the penultimate example of Consumer Driven Health Care. Traveling to another country for medical care is a solution that offers significant and immediate savings—often 90 percent—with almost no upfront investment and no reduction in quality of care if coordinated through a reputable medical tourism facilitator.</p>
<p>Until recently, medical tourism was fueled by individual patients. Now, employers pinched by 100 percent increases in health care premiums since 2000, are intrigued with alternatives that save money while expanding employee options, and for them, the arrow points to medical tourism. All parties involved in exercising this option win: employers save money on the cost of the procedure, employees save money by having the deductible waived or perhaps a reduction in out of pocket expenses. Employees also gain access to some of the most qualified surgeons in the world.</p>
<p><em><strong>&#8220;For employers, this is a &#8216;dare to be different NOW approach&#8217;,&#8221;</strong></em> said Wouter Hoeberechts, CEO of medical travel company WorldMed Assist. <em><strong>&#8220;Forward-looking businesses see a strong competitive advantage in a solution that retains employee loyalty through expanded health care options while at the same time shaving cost out of the business.&#8221;</strong></em></p>
<p>But not all businesses are so forward-looking. <em><strong>&#8220;Sadly, a more common approach is traditional cost-cutting by reducing benefits or shifting an ever-increasing portion of the burden and responsibility onto employees. Employees paid 17 percent more for insurance benefits in 2008 than in 2007,&#8221;</strong></em> said Hoeberechts. <em><strong>&#8220;This is a lose-lose-lose proposition: Employees lose precious benefits; businesses lose employee loyalty; and an ever-increasing number of Americans lose access to affordable health care. For long-term competitiveness, this strategy could become a death knell.&#8221;</strong></em></p>
<p>Politicians at both the state and federal levels have a clear mandate to fix access, affordability and efficiency of our health care system <em><strong>&#8220;The proposals on the table are varied, big, and expensive. Many of the solutions are excellent and are definitely needed, but they won’t deliver the savings on insurance premiums that employers and consumers need right now,&#8221;</strong></em> said Hoeberechts.</p>
<p>Impediments to speedy relief for skyrocketing health care costs are big: Stakeholders are lined up on all sides of all reform issues with swords drawn. With a national debt never before imagined, funding sources are bone dry. Building information technology platforms to facilitate electronic medical records, prescription improvements and enhancements to patient care coordination&#8211;all while protecting accuracy and consumer privacy – presents awesome hurdles.</p>
<p>Two other solutions gaining traction are a shift to preventative care to reduce acute care expense and waste, and paying for proven results rather than number of cases managed. <em><strong>&#8220;These will require entirely new paradigms from stem to stern within an industry that has a long tradition of embracing the status quo,&#8221;</strong></em> said Hoeberechts.</p>
<p>Consumers and employers can&#8217;t wait for consensus and funding solutions for such a gnarly array of issues. <em><strong>&#8220;For them, medical tourism is like an instant-win lottery ticket,&#8221;</strong></em> said Hoeberechts.</p>
<p>In working with employers and brokers to add a medical tourism component into their existing health plans, Hoeberechts explained his company can get a tailored plan up and running very quickly so that employers see immediate savings. <em><strong>&#8220;Once employers look into medical tourism, they find it hard to argue against a solution that’s available today, brings instant savings, retains employees and their loyalty, and expands medical options to include the world’s finest surgeons and facilities.&#8221;</strong></em></p>

			<content:encoded><![CDATA[<p><strong>BANGKOK &#8211; Many think Bangkok&#8217;s Bumrungrad Hospital invented medical tourism, but medical tourism didn&#8217;t invent Bumrungrad. The 1997-98 Asian economic crisis put Bumrungrad&#8217;s finances on the critical list and prompted its turn from local to international patients. Now the current recession has given Bumrungrad an opportunity to shore up the local foundation for its international business.</strong></p>
<p><span id="more-396"></span>Bumrungrad, the self-proclaimed largest private hospital in Southeast Asia, treated more than 420,000 international patients from nearly 200 countries last year.</p>
<p>That figure represented nearly 40% of Bumrungrad&#8217;s total of 1.1 million patients, and a similar percentage of Thailand&#8217;s medical tourism traffic. International patients, because they generally come for more complex treatments, accounted for more than 50% of Bumrungrad&#8217;s estimated US$260 million turnover.</p>
<p>A resident base of talented physicians and paying customers is vital to sustaining a world-class hospital, according to Bumrungrad&#8217;s chief executive officer Mack Banner. That limits the chances of success for destinations that hope to challenge Bangkok as a medical travel hub and it limits opportunities for international expansion by even the best-known hospital brands, Banner told Asia Times Online.</p>
<p>Banner, an American who retains his southern drawl and folksy charm despite more than 15 years overseas, has run the 554-bed hospital since mid-2005. Before Bumrungrad, Banner served as a healthcare executive in the US, Saudi Arabia, Australia, Singapore and Malaysia.</p>
<p><strong>Medical paradise</strong><br />
<em><strong>&#8220;I&#8217;ve been working overseas since the 1980s and I&#8217;ve had hundreds of proposals: &#8216;Let&#8217;s go to this nice location and put in a specialty hospital.&#8217; They never work. To have a top-flight hospital, you need top-flight doctors. These destinations don&#8217;t have a local base to develop that.&#8221; </strong></em>Banner explained. <em><strong>&#8220;The lesson I&#8217;ve learned is that hospitals need a critical mass of local population to support specialties that feed into sub-specialties that are needed to serve international patients.&#8221;</strong></em></p>
<p>Bangkok, the commercial and political capital of Thailand, has the right conditions, including <strong>&#8220;a large enough local population that can pay for your services to sustain a visitor base.&#8221;</strong> Thailand also has the advantage of a large base of medical doctors, many trained overseas. Prince Mahidol, father of the current Thai monarch, earned a medical degree at Harvard University &#8211; and married a Thai nurse who came to study there with him &#8211; helping to create a base of Western-trained doctors for teaching in Thailand. This traditional link to the royal family still thrives, providing a pipeline of quality healthcare personnel.</p>
<p>Bumrungrad has faced &#8220;a perfect storm of challenges&#8221; since September 2008, according to Banner. Those challenges included the H1N1 swine flu scare that, along with the global recession, crimped global travel, plus political unrest that climaxed with the shutdown of Bangkok&#8217;s Suvarnabhumi international airport for five days. Tourism to Thailand was hit particularly hard. Bumrungrad saw international visits drop 5.8% through the first three quarters of 2009, with international admissions down 8.5%, a sharp contrast to the double-digit growth of recent years.</p>
<p>Bumrungrad has been a household name in medical travel for more than a decade. <em><strong>&#8220;We enjoy a reputation as one of the pioneers of medical travel,&#8221; </strong></em>Banner said, noting that patients come from neighboring countries for better care and Western countries for cost savings, typically 50% or more. <em><strong>&#8220;Most of the attention [to medical travel to Thailand] has come from Bumrungrad. Now we share that spotlight, and we think that&#8217;s a good thing. But we&#8217;re still the first and we&#8217;re still the biggest.&#8221;</strong></em></p>
<p>To stay on top, Bumrungrad opened a new US$45 million outpatient building in May 2008, just ahead of the global recession that flattened growth last year. Ironically, the hospital&#8217;s first major expansion was completed in early 1997, months ahead of the Asian crisis. Financed with US dollar denominated loans, that project drove Bumrungrad to seek international patients, fueling the medical travel phenomenon. When fully operational, the new building will double outpatient capacity from 3,000 to 6,000 visitors daily.</p>
<p><em><strong>&#8220;We not only expanded capacity, we feel we expanded usability for patients,&#8221; </strong></em>Banner said. Innovations include pharmacy and cashier facilities on each floor, and imaging equipment to handle all but the most complex scans in the building. <em><strong>&#8220;We asked ourselves how to make the clinic experience as user-friendly as possible.&#8221;</strong></em></p>
<p><strong>Out of sight</strong><br />
By May, Bumrungrad will have completed a $12 million renovation of its inpatient rooms. Redesigned by a multi-disciplinary task force, the rooms feature bedside computer consoles for hospital personnel to access and enter medical information. Lower-tech improvements include more convenient sinks for hand-washing before patient contact.</p>
<p>Banner stressed that much of what gives Bumrungrad its competitive advantage remains out of sight. <strong>For example, a team of 20 handles up to 500 e-mail inquiries daily from prospective patients. They work with the International Medical Coordination Office, staffed by doctors and nurses to create treatment plans for patients in advance of their arrival.</strong></p>
<p>From its beginnings, Bumrungrad has been at the forefront of technological innovation. It established and evolved a single system to cover the entire hospital operation, a key factor in efficiently handling its outpatient volumes. That system was purchased by Microsoft in 2007 as the basis for its healthcare platforms, and Bumrungrad is working with Microsoft to update and revamp the software.</p>
<p>Last year, the hospital added an automated medication delivery system. At the heart of it, an Italian-made pharmacy robot &#8211; nicknamed Franco by staff &#8211; compiles and labels individual doses of inpatient prescriptions for delivery throughout the hospital via a pneumatic tube system. &#8220;We thought this through,&#8221; Banner explained, &#8220;How to make prescribing, administration and dispensing as simple as possible. It’s not a front of the house item, but it&#8217;s vital.&#8221;</p>
<p>Technology also helps Bumrungrad benchmark itself to international standards, but for Banner that&#8217;s not the most important measure. &#8220;How can we generate &#8216;wows&#8217;,?&#8221; is the key, according to Banner. He defines &#8220;wows&#8221; as &#8220;a service you don&#8217;t expect or one delivered in some exceptionally user friendly way.&#8221;</p>
<p>Bumrungrad is a private hospital listed on the Stock Exchange of Thailand, but Banner sees no conflict between patient care and profits. He cites an adage from Dr Thomas Frist, founder of Hospital Corporation of America (and father of former US Senate majority leader Bill Frist): &#8220;If you take care of patients, the bottom line will take care of itself.&#8221; Banner recalled, &#8220;I&#8217;ve worked for organizations that say, &#8216;We need a rate of return of X percent.&#8217; That&#8217;s short-term thinking that won&#8217;t work.&#8221; He cited the pharmacy robot as an example of a service improvement that can&#8217;t be quantified.</p>
<p>Bumrungrad gets a helping hand on keeping service up and costs down from its customers. Nearly three-quarters of its patients pay their own bills. &#8220;When people are paying their own bills, they act as their own utilization review committee,&#8221; Banner said. &#8220;If the doctor says you need an MRI, you ask, &#8216;what does it cost and where else can I do it that might cost less.&#8217; I&#8217;m a great believer in the effectiveness of self-pay to keep costs down. When you get into a third-party system, you engender a situation where costs can get out of control.&#8221;</p>
<p>That said, the hospital does accept payment from local and international insurers. It even has a deal with Blue Cross of South Carolina in the US that offers customers the option of having major procedures at Bumrungrad, with the patient sharing in the savings over a US hospital. However, this segment of the business, sometimes referred to as medical outsourcing, has been slow to develop. Banner hopes it will expand, but not at the cost of upsetting Bumrungrad&#8217;s winning formula.</p>
<p>&#8220;We&#8217;re not going to get into a situation where we&#8217;re second-guessed over pre-authorization and re-authorization that saddle hospitals with extra costs,&#8221; Banner said. &#8220;We already have a significant cost advantage over the US. We&#8217;ve told insurance companies to appreciate that and just let us do our jobs.&#8221;</p>

			<content:encoded><![CDATA[<p><strong>The Indian Government recently announced tourist visa on arrival for citizens of five countries &#8211; Japan, New Zealand and Singapore, in an effort to promote tourism. </strong></p>
